Consolidated Plan Annual Performance Report— FY 2001-2002 <br />City of San Leandro <br />Page 9 <br />Priority: Maintain, improve and expand services for homeless individuals <br />and families including integrated health care, employment services and other <br />supportive services. <br />Resources Used: <br />The City applied CDBG and HOME funds to further this priority. <br />Activities Undertaken: <br />Davis Street Community Center <br />0 The City provided CDBG and Community Assistance funding to the Davis Street <br />Community Center. The Center provides food, motel vouchers, and a medical clinic <br />to homeless people and those at risk of becoming homeless. <br />Building_ Futures with Women and Children (L3FWC) <br />n BFWC, Formerly known as San Leandro Shelter for Women and Shelter (SLSWC), <br />Received CDBG and Community Assistance funds for operational costs of the San <br />Leandro Shelter and the Safe House. <br />Shelter Against Violent Environments (Wings) <br />0 SAVE Received facility improvement funds for repairs to the transitional housing <br />apartments for survivors of domestic violence. <br />Geographic Distribution and Location of Expenditures: <br />Davis Street Community Center and BFWC are located in San Leandro; SAVE <br />transitional housing is located in Hayward. These agencies provide services on a regional <br />level. <br />Leverage of Non -Federal Resources: <br />The City Council annually allocates general funds to create the Community Assistance <br />Program. These funds are made available to non-profit agencies serving San Leandro <br />residents. <br />Matching Contributions: <br />N/A <br />Priority: Build on interjurisdictional cooperation and further coordination <br />and improvement of the homeless Continuum of Care System. <br />Resources Used: <br />The City applied CDBG and HOME funds to further this priority. <br />
