<br />Consulting Services Agreement between City of San Leandro and Last revised 06/07/2022 <br />Building Futures with Women and Children for Mobile Street Outreach Exhibit A – Page 1 of 4 <br />EXHIBIT A <br /> <br />SCOPE OF SERVICES <br /> <br />I. Definitions <br /> <br />Mobile Outreach: defined as a service delivered at sites and spaces where people experiencing <br />homelessness are located. This may include encampments, parking lots, bridge underpasses, tunnel <br />entrances, parks, sidewalks, and other locations in the community. Mobile Outreach aims to establish <br />supportive relationships and trust with homeless individuals through visits by Outreach Workers and <br />offering services and resources to end homelessness. Basic necessities to reduce harm and maintain <br />wellbeing may also be offered while individuals are living on the streets. <br /> <br />Harm Reduction Outreach: defined as an activity that promotes and addresses client and community <br />safety and wellness in the form of distribution of food, hygiene, blankets, cleaning materials and like items. <br />Harm Reduction Outreach also includes building relationships with clients, conducting intakes, and <br />providing opportunities for further case management. Activity occurs primarily in the field but will also <br />consist of maintenance of "drop-in hours" for basic supplies and for clients unable to be reached in the field <br />that week. This activity will be tracked by number of duplicated contacts per month. <br /> <br />Housing Problem Solving (HPS): Provide Housing Problem Solving services and supports to identify safe, <br />available alternatives to homelessness that can be secured with limited support, including administering <br />one-time flexible funding to assist with move-in expenses and other needs relevant to getting and keeping <br />housing. <br /> <br />Case Management: defined as an activity that guides, supports, and provides options for further se rvices <br />for individuals focused on housing goals and assistance with navigating the systems for achieving positive <br />housing outcomes. Case Management includes assessments, referrals, navigation, and advocacy. Active <br />case management clients will be defined as those who have had at least one face-to-face engagement with <br />the program no less than one time during a 90-day period. Case Management will take place in the field, <br />also called "street based," and in the central Building Futures offices, also called "of fice based," defined as <br />follows: <br /> <br />• Street-Based Case Management is for those most vulnerable, unable, unwilling, or otherwise not <br />engaged in office-based services. Service will be provided in the field where clients are located. <br />• Office-Based Case Management will focus on supporting clients' housing goals and offering <br />assistance with navigating the systems for achieving positive housing outcomes provided at Building <br />Futures offices. <br /> <br />II. Services to be Provided <br /> <br />Consultant shall provide mobile outreach and case management services in the City of San Leandro to a <br />minimum of 30 duplicated homeless individuals and families per week. Services will be conducted by a <br />Mobile Outreach Team comprised of one 0.5 FTE Housing Navigator and one 0.5 FTE Outreach Worker <br />funded by the City of San Leandro and additional staff funded by Alameda County Health Care Services. <br />Services shall be provided a minimum of 20 hours per week, spread over 4 days a week, of services will be <br />DocuSign Envelope ID: 124CBFDE-6C71-483B-BDE2-FFFAB7E62EEF
