EXHIBIT A - Page 2 of 2 <br />Delivery, will witness delivery of the food, and keep shared documentation of delivery for audit <br />purposes. <br />7.The City shall work with MBBP directly if any cancellations of food distribution and provide <br />advance notice as early as possible. <br />8.The City shall work with Mercy Brown Bag in registering qualified seniors on an annual basis <br />for the program, documenting participants in Link2Feed, and check pertinent information as <br />identified by MBBP prior to registration. <br />9.The City shall be responsible for accurate and timely recording of distributions to qualified <br />seniors in Link2Feed (within 5 working days of receipt of groceries). <br />10.Cooperate with MRCC to collect/recycle large brown grocery bags and large plastic grocery <br />bags for use in distribution of groceries. <br />11.The City Service Coordinators shall be responsible for routinely tracking the <br />volunteers that participate in MBBP distribution and services. This tracking <br />shall include the number of volunteers per month, the amount of time <br />contributed by volunteers for each date, and total amount of volunteer hours <br />contributed per month; volunteer contact information, such as email addresses <br />and/or phone numbers of volunteers, will be provided at the Coordinator's <br />discretion. This information shall be provided to MBBP on a monthly basis, via <br />a form that shall be provided to the site by MBBP. This information is due to <br />the External Operations Manager and/or the Volunteer Coordinator by the fifth <br />business day of the following month. <br />12.The City Service Coordinators are responsible for displaying our Code of <br />Conduct at every distribution (MBB shall provide) and ensuring the Code of <br />Conduct is upheld by recipients, volunteers, and employees. <br />DocuSign Envelope ID: B7D431A8-D5C5-4F91-8EE2-BFC745D33909
