Program impact and outcomes: <br />a. Number and percentage of unsheltered individuals who have exited the emergency <br />housing into year-round shelter or other interim housing <br />b. Number and percentage of unsheltered individuals who have exited the emergency <br />housing into permanent housing <br />c. Number and percentage of unsheltered individuals who have been connected to <br />employment assistance, full or part-time employment, general assistance, food <br />stamps, or other mainstream supports. <br />d. Other information that will help in the evaluation of the program <br /> <br />b) CONSULTANT shall participate in the evaluation of the program to help identify areas that will <br />improve service delivery, program effectiveness, and client outcomes. CONSULTANT will <br />implement program improvement strategies identified in the evaluation. <br />c) CONSULTANT shall submit a final narrative of the Emergency Housing Program by August 15, <br />2025, containing, at a minimum, the information in (a) above. The report shall include a narrative <br />accounting of the progress achieved toward the Scope of Work objectives, and the unsheltered <br />individual information in the monthly reports using aggregate reporting. <br />d) CONSULTANT shall make good faith efforts to collect and input data at 100% data quality. <br />e) CONSULTANT shall make good faith efforts to provide other information, as requested by the City <br />of San Leandro, in a timely manner. <br />For every participant, Consultant will: <br />• Provide the City of San Leandro with a dedicated point of contact for data quality and reporting (a <br />‘data lead’). <br />• Complete all entry/exit Universal Data Elements (UDEs) in HMIS. <br />• Provide real-time (within twenty-four hours) data entry of all participants served into the HMIS. At a <br />minimum, this includes the HMIS intake and project entries/exits, and services provided, as <br />applicable. <br />• Provide an HMIS “Annual Performance Report (APR)” report for the duration of the quarterly <br />period, reflecting the unduplicated total of persons served (including homeless persons or persons <br />at imminent risk of homelessness served). APR shall be extracted from the County’s HMIS and <br />shall not exceed 5% of null values in Questions 6a-f of the APR (Data Quality Sections). <br />APRs must be submitted for each individual project type (i.e., Rapid Re-housing, Homeless <br />Prevention, or relevant project) reports as demonstrated within the Quarterly Outcomes <br />Reporting: <br />1. Brief Description of Contract Services Specific to each quarter. <br />2. Number of Unduplicated Participants Served in each quarter. <br />3.
