<br />36 <br /> <br />Alameda Countywide Homeless and Special Needs Housing Plan <br /> <br />. <br /> <br />Increased rate of securing housing following discharge/release from a jailor other institution <br /> <br />Reductions in the number of school~age children who change schools or drop out as a result of <br />unstable housing or homelessness <br /> <br />. <br /> <br />2. People experiencing a crisis or in need of basic medical, behavioral health and/or social <br />services are able to access user-friendly and up-to-date information and obtain assessment <br />services through any provider of such services in the county. <br /> <br />Breaking through the barriers that have historically segmented service delivery is the primary <br />service-related change initiated by this plan. To the extent possible, service systems will develop <br />and utilize consistent definitions of the populations targeted in this plan, and at a minimum <br />clearly articulate eligibility criteria for services in each system. Through establishing consistent <br />quality of care standards, implementing approaches that treat the whole person, and developing <br />joint funding mechanisms, mainstream and specialized housing, income support, and human <br />service systems will offer the appropriate level of assistance to extremely low-income <br />households in need wherever they live, throughout the county. <br /> <br />Measures of progress and achievement might include: <br />· Reductions in the length of stay for families and individuals living in shelters <br />· Reductions in the number of teens leaving foster care who end up homeless <br />· Reductions in days someone is homeless <br />· Decreased lag time in receiving financial and/or medical benefits upon release from jail <br />· Reductions in the number of people in need of emergency health, mental health, and shelter <br />serVIces <br />· Increases in the number of T ANF recipients linked to housing assistance <br /> <br />3. People throughout Alameda County, including elected officials, community leaders, and <br />the general public demonstrate, through their charitable contributions, volunteer service, <br />funding decisions, and state and federal advocacy, an accurate understanding of how to <br />prevent homelessness and a solid commitment to remedy the complex social and health <br />issues faced by extremely low-income people living with HIV/AIDS, serious and persistent <br />mental illness, chemical dependency, and other disabling conditions. <br /> <br />Measures of progress and achievement regarding improved system performance and community <br />commitment might include: <br /> <br />· Increases in the number and type of formal interagency/inter-jurisdictional agreements <br /> <br />· Degree of blended funding and increases in the number of joint activities between and <br />among providers <br /> <br />· Extent to which staff from participating agencies are trained in each other's disciplines <br /> <br />· Degree to which program planning and development incorporates the participation of key <br />community stakeholders, including consumers <br /> <br />· Increases in the number of volunteer hours dedicated to ending and preventing homelessness <br />through faith- and community-based agencies across the county <br />
