Medical Transportation <br /> Reservation based (dial -a -ride) service is provided for trips for medical purposes only, within <br /> Alameda County to eligible riders. Rides are reserved on a first call, first served basis. <br /> Same -day reservations are to be accepted as available. <br /> Service Area: Alameda County <br /> Days of Operation: Monday - Friday <br /> Hours of Operation: 8:00 am — 5:00 pm <br /> Holidays: Service does not operate on approximately 10 City holidays/year <br /> Eligibility: Disabled Adults (18 +) and Seniors 75+ <br /> Reservations: Advance reservation <br /> Same Day Reservation: Accepted as available <br /> Fare: $4.00/10 Miles <br /> Fare Collection: Fare collected by driver & deducted from invoice <br /> Accessibility: Lift Equipped Wheelchair Accessible <br /> Number of Vehicles: (1) Bus <br /> Capacity: 8 Ambulatory, 2 Wheelchair <br /> Available Funding: Minimum Service Level Grant ($50,000 - $75,000) <br /> 2.0.4. Interim Service and Urgent Trips <br /> The CITY provides interim service or urgent trips on a case -by -case basis to eligible <br /> individuals who are not yet enrolled with East Bay Paratransit or the CITY Program for <br /> medical purposes. Interim service may include subscription rides. CITY authorization is <br /> required for interim service. <br /> Operating details are the same as Medical Transportation and the program is funded <br /> through the Minimum Service Level Grant. <br /> 2.1 ELIGIBILITY REQUIREMENTS <br /> The Flex Shuttle is intended for seniors and persons with disabilities. The CITY will determine eligibility <br /> criteria, assess applications and will electronically provide the CONTRACTOR with updated registration list. <br /> All registrants will be issued a registration card for presentation to shuttle bus drivers upon boarding. The <br /> CONTRACTOR will be responsible for the verification of all trip requests to ensure eligibility. <br /> Flex Shuttle Eligibility: <br /> • San Leandro Resident — Must live within the incorporated City limits. (CITY will determine <br /> eligibility) <br /> • 60 years old or older, or 18 years or older and East Bay Paratransit Certified <br /> Non - Professional Services Agreement between July 1, 2011 <br /> City of San Leandro and MV Transportation, Inc -- Exhibit A Page 2 of 16 <br />
