Medical Transportation Eligibility: • San Leandro Resident — Must live within the incorporated City limits. (CITY will determine eligibility) • 75 years old or older, or 18 years or older and East Bay Paratransit Certified. Non - registrant Companion: • Eligible passengers will be able to travel with a non - registrant companion to assist with parcels and act as a Personal Care Attendants (PCA) if the registrant feels one is required. All companions must be identified at the time a trip is requested. 2.2 CURRENT OPERATIONAL STATISTICS 2.2.1. Number of Registered Program Participants • Approximately 400 Registered Riders 2.2.2. Service Area • Flex Shuttle: City of San Leandro (Incorporated City Limits) • Medical Trips: Alameda County 2.2.3. Service Days • 5 days per week, Monday through Friday • • Approximately 10 holiday (non - service) days per year • Service on holidays is coordinated with CONTRACTOR in advance • Actual days of operation could change based on demand 2.2.4. Hours of Operation • Shuttle Bus: 9:00 AM to 3:00 PM • Medical Trips: 8:00 AM to 5:00 PM • Service will be provided during the hours listed above without a break in service for lunch or employee breaks 2.2.5. Approximate Billable Hours (Based on Grant & 2010 -11 Service) • Flex Shuttle Bus 3500 • Medical Trips 1988 • Interim Service (Included in Medical Trips) 2.2.6. Productivity — Average Number of Rides /Hour It is the goal of the program to maintain or increase the current high level of productivity for the Medical Service and to increase ridership on the Flex Shuttle Service. • Flex Shuttle Service: Current = 2 rides /hour, Goal = 8+ rides /hour Non - Professional Services Agreement between July 1, 2011 City of San Leandro and MV Transportation, Inc -- Exhibit A Page 3 of 16
