LACs have historically proven to be a key factor for a successful recovery and will typically <br />span into the long-term recovery phase. <br />Once the coordinatfon support of the EOC is no longer required, the EOC will demobilize personnel and <br />resources and officially deactfvate. The EOC Director determines demobilizatfon decisions and final deactfvatfon <br />in coordinatfon with the EOC Coordinator and Management Sectfon. <br />4.6. AFTER-ACTION REPORTING <br />The City of San Leandro conducts after-actfon reportfng actfvitfes following most incidents and exercises to <br />document strengths and opportunitfes for improvement from the event. The After-Actfon Report and <br />Improvement Plan (AAR/IP) provides an opportunity to capture what happened during the incident or exercise, <br />what went well and should be reinforced or repeated, and what gaps should be addressed. AAR/IPs typically also <br />include reviewing response actfons, suggestfng modificatfons to plans and procedures, and identffying training <br />and equipment needs to improve future responses. A correctfve actfon program provides reflectfve improvement <br />of emergency response capabilitfes. Correctfve actfons create the basis for the Improvement Plan, consistfng of <br />concrete, actfonable steps to resolve capability gaps identffied in the after-actfon review process. <br />To comply with SEMS requirements, the City of San Leandro must submit all after-actfon reportfng <br />documentatfon relatfng to real-world incidents to Cal OES within 90 days of the close of the incident period.31 <br />When describing the EOC actfvatfon in response to an incident, the AAR/IP will typically include response actfons <br />taken, applicatfon of SEMS, necessary modificatfons to plans and procedures, training needs, recovery actfvitfes <br />to date, and assignment of responsibilitfes in the Improvement Plan.
