LACs have historically proven to be a key factor for a successful recovery and will typically <br />span into the long-term recovery phase. <br />Once the coordina�on support of the EOC is no longer required, the EOC will demobilize personnel and <br />resources and officially deac�vate. The EOC Director determines demobiliza�on decisions and final deac�va�on <br />in coordina�on with the EOC Coordinator and Management Sec�on. <br />4.6. AFTER-ACTION REPORTING <br />The City of San Leandro conducts a�er-ac�on repor�ng ac�vi�es following most incidents and exercises to <br />document strengths and opportuni�es for improvement from the event. The A�er-Ac�on Report and <br />Improvement Plan (AAR/IP) provides an opportunity to capture what happened during the incident or exercise, <br />what went well and should be reinforced or repeated, and what gaps should be addressed. AAR/IPs typically also <br />include reviewing response ac�ons, sugges�ng modifica�ons to plans and procedures, and iden�fying training <br />and equipment needs to improve future responses. A correc�ve ac�on program provides reflec�ve improvement <br />of emergency response capabili�es. Correc�ve ac�ons create the basis for the Improvement Plan, consis�ng of <br />concrete, ac�onable steps to resolve capability gaps iden�fied in the a�er-ac�on review process. <br />To comply with SEMS requirements, the City of San Leandro must submit all a�er-ac�on repor�ng <br />documenta�on rela�ng to real-world incidents to Cal OES within 90 days of the close of the incident period.31 <br />When describing the EOC ac�va�on in response to an incident, the AAR/IP will typically include response ac�ons <br />taken, applica�on of SEMS, necessary modifica�ons to plans and procedures, training needs, recovery ac�vi�es <br />to date, and assignment of responsibili�es in the Improvement Plan.
